1. Eligibility
• Patients must be 18 years or older or accompanied by a legal guardian to receive services. Minors require parental or guardian consent for treatment.
• Services are available to individuals seeking podiatric care, subject to medical evaluation and acceptance by our providers.
2. Appointments
• Scheduling: Appointments can be made by phone at (941) 917-6232, online at www.sarasotafootcarecenter.com, or in person. Accurate contact information is required for confirmations and reminders.
• Cancellations and No-Shows: A minimum of 48 hours’ notice is required for cancellations or rescheduling. Failure to notify may result in a no-show fee of $75, except in cases of documented emergencies.
• Late Arrivals: Patients arriving more than 15 minutes late may need to reschedule, subject to availability, to ensure timely care for all patients.
3. Payment and Insurance
• Payment Responsibility: Patients are responsible for all charges for services rendered. Payment is due at the time of service unless prior arrangements are made.
• Payment Methods: We accept cash, credit/debit cards (Visa, MasterCard, American Express), and checks. Contact our office for details on accepted payment methods.
• Insurance: We accept most major insurance plans. Patients must verify cover age with their provider and provide accurate insurance information. Copays and deductibles are due at the time of service.
• Uninsured Patients: Self-pay rates are available and will be discussed prior to treatment. Payment plans may be offered at our discretion.

5. Patient Responsibilities
• Accurate Information: Patients must provide complete and accurate medical history, contact details, and insurance information to ensure safe and effective care.
• Treatment Compliance: Adherence to prescribed treatments, follow-up appointments, and care instructions is essential for optimal outcomes. Non-compliance may affect treatment results.
• Respectful Conduct: We maintain a respectful environment. Abusive or disruptive behavior toward staff or other patients may result in termination of care.
6. Privacy and Confidentiality
• We comply with the Health Insurance Portability and Accountability Act (HIPAA) to protect your personal and medical information. Your data will only be shared as permitted by law or with your consent.
